Would you like to register a patient for an Exploratory Meeting?
Registration is possible via a referral to Huisarts+punt in ZorgDomein:
- Choose GGZ or Welzijn → overige zorgvragen/leefstijl algemeen
- Type Huisartspluspunt or Verkennend Gesprek in the search bar
- The consultation options will appear at the top
