This conversation aims to provide an in-depth assessment of mental health, considering both mental health care needs and potential challenges in other areas of life. The conversation takes place at one of the GP practices in Amsterdam Zuidoost, is face to face, and consists of 2 appointments of 60 minutes each, followed by a telephone follow-up. After the conversations, the referrer will receive feedback.
